Job Description:
A company is looking for a Patient Navigator - Post Acute Care
Continuum.
Key Responsibilities
Collaborates with post-acute facilities to optimize patient
navigation through the care continuum
Identifies patients not meeting clinical goals and arranges
appropriate follow-up
Reviews and updates medication lists and documents allergies in the
electronic health record at discharge
Required Qualifications
Specialized Diploma Registered Nurse or Master's Degree in Physical
or Occupational Therapy
3 years of experience in Post-Acute Care
3 years of experience with Electronic Medical Records (EMR)
Current licensure as a Registered Nurse, Physical Therapist, or
Occupational Therapist in Pennsylvania
